I am no expert, but it looks like Leaning Crown Syndrom (LCS) which is most likely caused by boron deficiency (as far as I know).

Yup, severe boron deficiency.  There's a queen palm around here that looks pretty much like that poor thing.  I've been watching the hotel lose palm after palm after palm to Thielaviopsis (with secondary Ganoderma infection) but somehow the insanely distorted queen just keeps on growing.  I doubt they have treated it for the boron deficiency, they are just letting it slowly die.  Here's more info on boron:
