Pinto problems

I planted this Pindo in March, balled and burlap 3ft or so of trunk. 
Planted properly, big tree ring to hold water and watered 2 times a month, on the 1st and 15th.  
I did that watering schedule for March-July and then went to once a month in August til now. 
 

I did that water schedule because I just replaced a Pindo in the exact same spot that died due to over watering. 
 

Palm has been doing great. I recently trimmed 5 completely brown lower branches so I thought it was doing it’s thing until 2 fronds from the middle spear completely fell literally overnight. 
I tugged on the middle spear and it didn’t pull out. 
 

Any ideas, this is terrible.

could be a root problem like root bugs or fungus.

Photos of the growing point showing the newest fronds and spear would be helpful to see.

Check the soil underneath the rocks. Rocks can trap a ton of water in the soil.

Watering is best done according to the weather and the needs of the plant, not the calendar.
